The Macon County Health Department is
licensed by the Illinois Department of Public Health as a Home Nursing Agency
and offers Medication Management Services for persons of all ages.
We are funded by the East Central Illinois Area Agency on
Aging to provide Medication Management Services to persons 60 and over who
qualify for the program.
We also provide private pay services to persons of ALL ages
who are in need of assistance with their medications. Private visits are
available for $50 per visit and frequency is based on individual needs.
Our services are provided by a Registered Nurse who makes
home visits and assists in any way possible to assure medications are managed
appropriately. We work with physicians, clinics and pharmacies to support
individuals with their medication management, and help improve their ability to
live independently
Services provided include:

Set-up with a registered nurse either weekly or bi-weekly (frequency is
based on need),

Fill medication organizers,
-
Arrange visits for medication changes,

Reorder medications,
-
Contact pharmacies and arrange for prescription deliveries or pick up,

Provide education on current medications and chronic diseases,
-
Monitor compliance of medications,
-
Monitor changes in health conditions,
-
Assist in monitoring blood sugars, weight, and blood pressure,

Pre-fill insulin syringes,
-
Obtain annual health history,
-
Contact physician every 90 days,
-
Screen for psychosocial problems, safety/environmental issues, elder abuse,
and self-neglect, and
-
Referral to Starting Point (Aging and
Disability
Resource
Center) Care Coordinators whom are available to
every client at no charge for other in-home needs.
